Our client based in Bournemouth is seeking a motivated and experienced Adult Social Worker to join their Preparing for Adulthood (PfA) Team. This is a key role supporting young people as they transition from Children’s Services into Adult Social Care, ensuring seamless, person-centred planning and positive long-term outcomes.
The team works with young people with complex needs, including those with Education, Health and Care Plans (EHCPs), and requires a practitioner with strong knowledge of legislation, excellent assessment skills, and the ability to work confidently across multi-agency settings.
Responsibilities

- →Supporting young people through transition from Children’s Services to Adult Social Care, ensuring continuity and effective planning.
- →Undertaking Care Act 2014 assessments and reviews, ensuring compliance with statutory responsibilities.
- →Completing Mental Capacity Assessments (MCA) and confidently applying Best Interest decision-making frameworks.
- →Developing strengths-based, person-centred support plans that promote independence and successful adulthood outcomes.
- →Managing a complex caseload, including young people with EHCPs and additional needs.
- →Preparing high-quality written reports, including court reports and statements where required (including Court of Protection work).
- →Working collaboratively within a multi-agency framework, including education, health professionals, and partner agencies.
- →Accurately recording casework, assessments, and plans using Mosaic (or equivalent case management systems).
- →Applying sound risk management and professional judgement within legal and policy frameworks.
- →Building strong relationships with young people, families, and carers to support effective engagement and outcomes.

- Registration: Must be registered with Social Work England.
• Experience:
- Proven experience working within Adult Social Care.
- Experience supporting young people in transition (Preparation for Adulthood / PfA).
- Experience working with individuals with EHCPs and complex needs.
- Experience using Mosaic or equivalent systems for case recording and care planning.
- Strong experience in producing analytical, high-quality written reports, including legal documentation (desirable for Court of Protection work).
• Legislation Knowledge:
- Strong understanding of the Care Act 2014 and Mental Capacity Act 2005.
- Experience completing Mental Capacity Assessments and Best Interest decisions.
- Knowledge of transition processes and statutory responsibilities for young adults.
- Excellent assessment, analytical, and decision-making skills.
- Strong written and report-writing ability, including legal documentation.
- Ability to manage complex caseloads effectively.
- Excellent communication and interpersonal skills.
- Ability to work confidently across multi-agency environments.
