Adult Social Worker – RIACT

Rapid Response Assessments:

• →Conduct urgent assessments of adults experiencing a sudden decline in health or social functioning.
• →Determine eligibility for care and support under the Care Act 2014.
• →Identify and address immediate needs.

Crisis Intervention and Hospital Avoidance: 

• →Prevent unnecessary hospital admissions by arranging short-term care packages, reablement services, or community support.
• →Support timely hospital discharges by coordinating care plans for safe return home.

Safeguarding and Multi-Agency Coordination:

• →Identify and respond to safeguarding concerns, including referrals and strategy meetings.
• →Collaborate with health professionals, occupational therapists, and care providers to deliver integrated care.

To be successful in this role you must have:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
