Liquid Personnel is seeking a dedicated and experienced Adult Advanced Practitioner to join our client’s Hospital Team in Warrington.
Responsibilities

- →Lead complex discharge planning and assessments, ensuring safe transitions of care from hospital to community services, with a focus on timely interventions and reducing delayed discharges.
- →Provide expert guidance and decision‑making support to social workers and multi‑disciplinary colleagues, particularly around safeguarding, Mental Capacity Act, and Continuing Healthcare considerations.
- →Coordinate with clients, families, and professionals in a fast‑paced hospital environment, balancing clinical priorities with social care responsibilities to deliver integrated, person‑centred outcomes.

- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
