Advanced Practitioner – Intermediate Care

Liquid Personnel is recruiting an experienced Advanced Practitioner to join its client’s Intermediate Care service based in Somerset. This is a full‑time agency role supporting complex casework, hospital discharge pathways, and admission avoidance within a multi‑disciplinary environment.

• →Lead on complex assessments, including Care Act assessments, discharge planning and risk assessments
• →Support timely hospital discharges and admission avoidance through effective intervention planning
• →Manage a complex caseload, providing expert guidance on high-risk and high-need cases
• →Develop and review person-centred care and support plans that promote independence and reablement
• →Undertake safeguarding enquiries and ensure risk is managed appropriately
• →Apply the Mental Capacity Act, including Best Interests decision-making
• →Provide professional support, mentoring and guidance to less experienced staff
• →Work closely with acute hospitals, community health teams and wider multi-agency partners
• →Prepare clear, high-quality reports and maintain accurate case records
• →Support service development and contribute to best practice within the team
