Responsibilities
~1 min read- →Carry out functional assessments for adults with physical disabilities, frailty, or complex needs
- →Prescribe equipment to promote safe moving & handling and independence
- →Make recommendations for minor and major home adaptations
- →Collaborate with physiotherapists, social workers, and external partners
- →Maintain accurate, timely clinical documentation
- →Prioritise caseloads effectively in line with service demands
- →Contribute to safeguarding and risk assessments where required

~1 min readTo be successful in this role you must have,
- HCPC‑registered Occupational Therapist
- Experience in community OT practice
- Strong knowledge of moving & handling, equipment prescription, and adaptations
- Ability to complete clinical recording and reporting to high standards
- Car driver desirable but not essential
