We are seeking an experienced Occupational Therapist to join the Adult Social Care team. This is an exciting opportunity to make a real difference by supporting individuals to live safely and independently in their own homes. You will work within a supportive multidisciplinary environment, delivering high-quality, person-centred interventions.
Responsibilities
~1 min read- →Complete Care Act-compliant functional assessments for adults with physical disabilities, long-term conditions, and complex needs.
- →Develop and implement reablement plans, promoting independence and reducing reliance on long-term care.
- →Advise on major and minor adaptations, including Disabled Facilities Grants (DFG), and recommend assistive technology solutions.
- →Undertake Moving & Handling risk assessments and provide training to carers and staff.
- →Support hospital discharge pathways through timely assessments and coordination of community services.
- →Work collaboratively with social workers, physiotherapists, housing teams, and other professionals to deliver holistic care.
- →Maintain accurate case records and produce reports for panels and funding applications.

~1 min readTo be successful in this role you must have,
- HCPC registration as an Occupational Therapist.
- Previous experience in Adult Social Care within a local authority or NHS setting.
- Strong knowledge of the Care Act, safeguarding, and DFG processes.
- Ability to manage a varied caseload and work autonomously.
- Car driver with business insurance (essential for community visits).
