Liquid Personnel is recruiting for an experienced Occupational Therapist to join Wiltshire Council's Reablement Team, supporting adults following hospital discharge and helping them regain independence.

This is a fantastic opportunity to work within a supportive team delivering person-centred assessments and reablement interventions across home, hospital, and community settings.

Responsibilities

→Complete Occupational Therapy assessments following hospital discharge
→Develop tailored reablement plans to maximise independence
→Provide advice on equipment, moving safely, and best practice
→Work collaboratively with colleagues to support decision-making and positive outcomes
→Carry out community visits while managing a varied caseload

Requirements

To be successful in this role you must have,

Qualified Occupational Therapist
Minimum 2 years' post-qualification experience
Full UK driving licence
