Qualified Social Worker

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s team, based in Conwy. 

What will your responsibilities be? 

• Hospital discharge planning — Lead safe and timely discharges by completing strengths‑based assessments, coordinating MDT input, and arranging appropriate community or residential support in line with the Social Services and Well‑being (Wales) Act 2014.

• Adult safeguarding — Identify, investigate, and manage safeguarding concerns for older adults and vulnerable adults, undertaking Section 126 enquiries, risk assessments, and protection planning with partner agencies.

• Complex case management — Hold a varied caseload including hospital‑based referrals, cognitive decline, physical frailty, and long‑term conditions; complete Care and Support Plans, Mental Capacity Act assessments, and best‑interest decisions.

Qualifications and Experience: 

To be successful in this role you must have;  

• Social Work England registration 
• Eligible to work in the UK 
• Hold a full UK driving licence 
• Post-qualified experience 
• Degree level or equivalent in Social Work
