Social Worker - CHC

Liquid Personnel is recruiting an Adult Social Worker to join its client’s CHC team, based in Somerset. This is a key opportunity to contribute to high-quality Continuing Healthcare (CHC) assessments and support robust, evidence-based decision making within a multi-disciplinary environment.

Qualifications and Experience: 

To be successful in this role you must have;  

• →Qualified Social Worker with current professional registration.
• →Extensive experience completing CHC assessments and DSTs.
• →Strong understanding of the CHC National Framework.
• →Detailed knowledge of the Care Act 2014, particularly Section 22.
• →Excellent communication, analytical and stakeholder management skills.
• →Ability to work effectively within a multi-disciplinary team.
