Liquid Personnel is recruiting an Adult's Social Worker to join its client’s Community Team, based in Barking and Dagenham. 

What will your responsibilities be? 

  • Manage a caseload of adults with varying levels of need, including complex cases depending on experience.
  • Complete Care Act assessments, reviews, and reassessments.
  • Develop and review person-centred support plans that promote independence and wellbeing.
  • Undertake safeguarding enquiries and manage risk effectively.
  • Complete Mental Capacity Assessments and apply the Mental Capacity Act where appropriate.
  • Support hospital discharges and work proactively to prevent unnecessary admissions.
  • Work collaboratively with individuals, families, health professionals, community partners, housing providers, voluntary organisations, and other partner agencies.
  • Provide information, advice, and guidance to adults and their carers.
  • Coordinate care and support to ensure individuals receive appropriate services and achieve positive outcomes.
  • Support vulnerable adults while promoting informed choice, independence, and wellbeing.
  • Maintain accurate, timely, and high-quality case records.
  • Contribute to service development and continuous improvement initiatives.
  • Support and supervise less experienced colleagues where appropriate.

Qualifications and Experience: 

To be successful in this role you must have;  

  • A recognised Social Work qualification
  • Current Social Work England (SWE) registration
  • Experience undertaking assessments, reviews, and safeguarding interventions within adult social care
  • Good knowledge of the Mental Capacity Act and relevant legislation
  • Strong multi-agency partnership working skills
  • Excellent communication, organisational, and problem-solving abilities
  • A compassionate, proactive, and person-centred approach to practice
  • A commitment to delivering high-quality outcomes for adults and their families
