Social Worker - Community Team

Liquid Personnel is recruiting Social Worker for its client's Community Team at Slough.
This is an excellent opportunity for a dedicated practitioner to work with adults and their careers, delivering strengths-based, person-centred social work interventions that promote independence, wellbeing, and positive outcomes.
The successful candidate will hold a caseload of adults and/or their careers, undertaking assessments, care and support planning, reviews, safeguarding interventions, and risk management. Working within a community setting, you will apply a strengths-based approach to support individuals in achieving their desired outcomes whilst maintaining independence and maximising available community resources and support networks.
You will work collaboratively with individuals, families, carers, health professionals, providers, and partner agencies to ensure people receive appropriate and proportionate support in accordance with relevant legislation, departmental procedures, and best practice.
Responsibilities
~2 min read- →Managing a caseload of adults and carers, undertaking assessments and interventions in accordance with statutory duties and best practice.
- →Completing strengths-based Care Act Assessments to identify eligible needs, personal outcomes, and appropriate support arrangements.
- →Developing, implementing, and reviewing person-centred care and support plans that promote independence, wellbeing, and community inclusion.
- →Undertaking scheduled and unscheduled reviews to ensure support arrangements remain appropriate and responsive to changing needs.
- →Working collaboratively with individuals, carers, families, advocates, and partner agencies to identify strengths and maximise community-based support.
- →Assessing and managing risk, ensuring appropriate interventions are implemented to safeguard vulnerable adults and promote positive outcomes.
- →Responding to safeguarding concerns and undertaking safeguarding enquiries in line with the Care Act 2014 and local safeguarding procedures.
- →Undertaking Mental Capacity Assessments where required and applying the principles of the Mental Capacity Act 2005.
- →Supporting individuals through periods of crisis and increased vulnerability, utilising professional judgement and available resources to achieve positive outcomes.
- →Coordinating services and support provision with health colleagues, care providers, voluntary sector organisations, and community resources.
- →Undertaking carers' assessments and ensuring carers receive appropriate support and access to relevant services.
- →Preparing clear, concise, and evidence-based assessments, reports, support plans, and case recordings.
- →Maintaining accurate and up-to-date case records in accordance with legislative, professional, and organisational standards.
- →Participating in multi-disciplinary and multi-agency meetings to ensure coordinated and effective support for individuals and their families.

- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
