Social Worker - Hospital Discharge

• →Manage a hospital-based social work caseload.
• →Complete assessments and support planning to facilitate safe discharges.
• →Work closely with NHS professionals, patients, families, and partner agencies.
• →Coordinate care packages and community support to prevent delayed discharges.
• →Undertake safeguarding responsibilities and risk assessments where required.
• →Maintain accurate and timely case records.
• →Provide cross-site cover across multiple hospital locations as required.
• →Participate in weekend working on a rota basis.

To be successful in this role you must have, 

• Qualified Social Worker (BA, MA, DipSW, CQSW or equivalent).
• Registered with Social Work England.
• Previous hospital social work experience is essential.
• Experience supporting hospital discharge planning.
• Strong assessment, communication, and multi-agency working skills.
• Ability to work flexibly across hospital sites and weekends.

• Minimum of 3 years' references.
• Full employment history with any gaps explained.
• Relevant qualifications in line with the role requirements.
