Social Worker - Hospital Discharge

Responsibilities
~1 min read- →Manage a hospital-based social work caseload.
- →Complete assessments and support planning to facilitate safe discharges.
- →Work closely with NHS professionals, patients, families, and partner agencies.
- →Coordinate care packages and community support to prevent delayed discharges.
- →Undertake safeguarding responsibilities and risk assessments where required.
- →Maintain accurate and timely case records.
- →Provide cross-site cover across multiple hospital locations as required.
- →Participate in weekend working on a rota basis.
Requirements

- Qualified Social Worker (BA, MA, DipSW, CQSW or equivalent).
- Registered with Social Work England.
- Previous hospital social work experience is essential.
- Experience supporting hospital discharge planning.
- Strong assessment, communication, and multi-agency working skills.
- Ability to work flexibly across hospital sites and weekends.
- Minimum of 3 years' references.
- Full employment history with any gaps explained.
- Relevant qualifications in line with the role requirements.
