Social Worker - Hospital Discharge Team

Liquid Personnel is seeking an experienced Social Worker to join its clients Hospital Discharge Team, supporting citizens across discharge pathways. 

• →Assess and support citizens on discharge from hospital across three pathways
• →Complete post-discharge assessments and reviews
• →Undertake Care Act Assessments (CAA) and support planning
• →Carry out Best Interest (BI) decisions and Mental Capacity Act (MCA) work
• →Lead on safeguarding enquiries and interventions
• →Provide duty cover as required
• →Attend Quality Assurance panels for funding provisions
• →Support Discharge to Assess (D2A) pathways

• Proven experience within hospital discharge settings
• Strong working knowledge of Care Act, MCA, safeguarding, and discharge processes
• Ability to manage a fast-paced caseload and work under pressure
• Excellent assessment and decision-making skills
• Available to start immediately

To be successful in this role you must have, 

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
