Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Learning Disabilities team, based in Havering.

What will your responsibilities be?

Manage a diverse caseload of adults with learning disabilities, including complex cases depending on experience.
Work closely with adults, carers, healthcare professionals, and community partners to deliver positive and lasting outcomes.
Undertake person-centred assessments, reviews, and support planning.
Complete safeguarding enquiries and risk assessments to protect vulnerable adults.
Promote independence and ensure individuals receive the right support at the right time.
Support safe hospital discharges and work to prevent avoidable admissions.
Develop and review personalised care and support plans.
Work collaboratively within a multi-disciplinary team alongside health and social care professionals.
Maintain accurate and timely case records and reports.
Contribute to service development and continuous improvement initiatives.
Provide mentoring or supervision support to colleagues where appropriate.

Qualifications and Experience:

To be successful in this role you must have;

A recognised Social Work qualification
Current Social Work England (SWE) registration
Experience undertaking assessments, reviews, and safeguarding interventions within adult social care
Good knowledge of the Mental Capacity Act and relevant legislation
Strong multi-agency partnership working skills
Excellent communication, organisational, and problem-solving abilities
A compassionate, proactive, and person-centred approach to practice
A commitment to delivering high-quality outcomes for adults and their families
