Our client based in Shropshire is seeking an experienced Mental Health Social Worker to join their dedicated Mental Health Team. This is an excellent opportunity for a skilled practitioner to support adults with complex mental health needs, promoting recovery, independence, and positive outcomes through strengths-based and person-centred practice.

In this rewarding role, you will manage a varied and complex caseload while working closely with health professionals, hospital teams, community mental health services, families, carers, and partner agencies. You will play a key role in supporting timely hospital discharges, undertaking statutory social work duties, coordinating aftercare services, and ensuring individuals receive the right support to live safely and independently within their communities.

Responsibilities

  • Managing a caseload of adults with complex mental health needs, completing assessments, interventions, reviews, and support planning.
  • Undertaking Care Act 2014 assessments and reviews to identify eligible needs and develop outcome-focused care plans.
  • Applying the Mental Capacity Act 2005, including Mental Capacity Assessments and Best Interest decision-making.
  • Working within the Mental Health Act 1983/2007 framework and supporting individuals subject to statutory interventions.
  • Facilitating timely hospital discharge planning and addressing barriers to discharge.
  • Coordinating Section 117 Aftercare Planning and Reviews.
  • Arranging care packages, supported accommodation, residential placements, and community-based support services.
  • Undertaking safeguarding enquiries and responding to concerns relating to abuse, neglect, self-neglect, and exploitation.
  • Working collaboratively with Community Mental Health Teams, NHS partners, housing providers, carers, and voluntary sector organisations.
  • Assessing and managing risk effectively, ensuring robust and defensible decision-making.
  • Maintaining accurate, timely, and legally compliant case records, assessments, and reports.
  • Social Work qualification (BA, BSc, MA, Diploma in Social Work, CQSW or equivalent).
  • Current registration with Social Work England.
  • Significant post-qualified experience within Adult Mental Health Services.
  • Strong knowledge of the Care Act 2014, Mental Health Act 1983/2007, Mental Capacity Act 2005, and Adult Safeguarding legislation.
  • Experience completing Care Act assessments, Mental Capacity Assessments, risk assessments, and support planning.
  • Experience supporting complex hospital discharges and community-based interventions.
  • Knowledge of Section 117 Aftercare arrangements and reviews.
  • Excellent communication, assessment, report-writing, and decision-making skills.
  • Full UK driving licence and access to a vehicle desirable.
