Social Worker – Older People's Team

Liquid Personnel is recruiting an experienced Locum Adult Qualified Social Worker for its client’s Older Persons Team based in Wrexham. This is a full-time agency role offering £33.42 per hour, supporting older adults to maintain independence and achieve positive outcomes through strengths-based social work practice.

• →Manage a caseload of older adults, completing strengths-based assessments, care planning, reviews, and interventions to promote independence and achieve positive outcomes.
• →Undertake Mental Capacity Assessments, Best Interest decisions, safeguarding enquiries, and risk assessments, ensuring compliance with relevant legislation and professional standards.
• →Work collaboratively with families, carers, health professionals, and partner agencies, while participating in the duty rota to respond to referrals, urgent enquiries, and priority cases across community and hospital settings.

• Social Work degree or equivalent qualification (BA Social Work, MA Social Work, DipSW, CQSW).
• Registered with Social Care Wales.
• Experience working within Adult Social Care, ideally with older adults.
• Knowledge of the Mental Capacity Act, safeguarding procedures, and adult social care legislation.
• Ability to complete assessments, care planning, reviews, and risk assessments.
• Strong communication and partnership-working skills.
• Ability to manage a caseload effectively and work as part of a multidisciplinary team.
• Enhanced DBS check (or willingness to obtain one).
