Responsibilities

→Provide neurological evaluations and follow-up care for nursing home residents.
→Diagnose and manage conditions such as stroke recovery, Parkinson’s disease, dementia, neuropathy, and other neurological disorders.
→Collaborate with nursing home staff, families, and interdisciplinary teams to create comprehensive care plans.
→Document all patient encounters and maintain compliance with healthcare regulations.
→Educate patients, caregivers, and facility staff on neurological conditions and treatments.

Requirements

Licensed Nurse Practitioner (NP) in New York State.
Board Certification in Neurology or experience in neurology preferred.
Minimum 1-2 years of NP experience (geriatrics or neurology a plus).
Experience in long-term care or nursing home settings preferred.
Strong clinical judgment and the ability to work independently.
Excellent communication and documentation skills.
