Job Summary: The Day Hab Navigator is responsible for assisting individuals in acquiring and maintaining the skills, personal habits, and positive attitudes necessary for enhancing their independence and quality of life. This position involves providing structured services and skill-building activities, conducting outreach, and connecting clients to community resources. The Day Hab Navigator will work closely with clients to develop personalized plans that address their needs and goals.
Key Responsibilities:
- Client Support: Assist clients in developing and implementing day hab. plans that reflect their interests, capabilities, and needs. Provide prompts, reminders, guidance, and role modeling in areas such as socialization, personal hygiene, exercise, nutrition, and behavior.
- Coordinates Services: Work with individuals, coordinates activities with other departments, medical providers, social workers, schools and/or other outreach agencies as appropriate to ensure that the needs of the clients are met.
- Skill Building: Develop and provide structured services, activities, behavior modification and social skills to clients as part of rehabilitation skills; implements wellness and recovery philosophy and values to clients through education and outreach program, enhancing their skills and level of independence.
- Community Integration: Encourage clients to participate in community activities and events, fostering a sense of belonging and integration.
- Outreach: Conduct outreach in the community to engage clients and connect them to resources. Meet clients in various settings, including shelters, parks, and community events.
- Advocacy: Advocate on behalf of clients to ensure they receive the necessary support and services. Assist clients with applications for public benefits and other resources.
- Documentation: Maintain accurate records of client interactions, progress, and services provided. Use electronic health records systems to document all contacts and services.
- Safety and Welfare: Ensure the safety and welfare of clients during activities
- Team Collaboration: Work collaboratively with other staff members, managed care plans, primary care providers, and local programs to coordinate services and support for clients.
Qualifications:
- High school diploma or equivalent.
- Experience working with individuals with disabilities or in a similar role preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Basic computer skills, including proficiency in Microsoft Office and electronic health records systems.
- Level 1: Day Hab Navigator: High School Diploma or GED equivalent – Entry level position- will hold caseload of up to 20 clients. Will facilitate adult workgroups in Center.
- Level 2: Day Hab Resource Advocate (2) years of full-time experience performing advanced journey level or specialized clerical duties in an office environment. Should have experience performing duties comparable to a Case Manager. Case studies or college credits for Psychology, Sociology is a bonus but not required. Will hold caseload of 21-40 clients while facilitating adult workgroups in Center. Will perform outreach services for new clients.
- Level 3: Community Health Specialist (3) years of full-time experience performing advanced journey level or specialized Case Manager in an office environment. Should have experience performing duties comparable to a Case Manager. Case studies or college credits for Psychology, Sociology is required. Hold caseload of 41-60 clients while facilitating adult workgroups in Center. Will perform outreach services for new clients.
Working Conditions:
- Schedule 35 hours per week, Monday through Friday with some flexible hours, including early mornings, evenings, weekends, and holidays as needed.
- Ability to stand for long periods and perform repetitive tasks.
- Comfortable working in various community settings and environments.
Benefits:
- Salary depends on experience and qualifications. This position performs a variety of para-professional, responsible eligibility determination, case management duties, and performs related duties as assigned. The position is under New Leaf Industries, a division of the Ministerial Association of California Counties and is scheduled for thirty-five (35) hours per week.
