Position Summary
The Family Practice Physician provides comprehensive, patient-centered primary care services to individuals and families across the lifespan within a Federally Qualified Health Center Look-Alike (FQHC Look-Alike) setting. This role is dedicated to serving a diverse and often underserved population, with a strong emphasis on preventive care, chronic disease management, and whole-person health. The Physician works collaboratively within an interdisciplinary care team to deliver high-quality, culturally responsive care aligned with FQHC Look-Alike mission and values.
Key Responsibilities
- Provide full-spectrum primary care services, including diagnosis, treatment, and management of acute and chronic conditions for patients of all genders and ages
- Deliver preventive care services, including annual exams, immunizations, screenings, and health education
- Manage chronic diseases such as diabetes, hypertension, and asthma using evidence-based guidelines
- Perform routine in-office procedures
- Provide women’s health services, including contraceptive counseling
- Coordinate care with behavioral health, dental, and specialty providers as part of an integrated care model
- Utilize electronic health records (EHR) to document patient encounters accurately and timely
- Participate in care team huddles, case conferences, and quality improvement initiatives
- Support population health efforts, including outreach and care gap closure
- Provide culturally competent, trauma-informed care to a diverse patient population
- Participate in after-hours call rotation as needed
FQHC Look-Alike-Specific Expectations
- Demonstrate commitment to serving underserved and vulnerable populations
- Learn, understand, and support FQHC Look-Alike compliance requirements, including UDS reporting and HRSA guidelines
- Contribute to achieving quality metrics and value-based care goals
- Maintain productivity expectations while prioritizing quality and patient-centered care
- Ensuring continuity of care
Minimum Qualifications
- MD or DO from an accredited medical school
- Completion of an accredited Family Medicine residency program
- Board certified or board eligible in Family Medicine
- Active and unrestricted medical license in WI
- DEA certification
- Basic Life Support (BLS) certification
Preferred Qualifications
- Prior experience working in an FQHC, community health center, or underserved setting
- Bilingual skills
- Experience with integrated care models (behavioral health, dental coordination)
- Familiarity with value-based care and quality metrics

Work Environment & Schedule
- Outpatient clinic setting within an FQHC Look-Alike
- Team-based care model with support staff
- Full-Time - 4 days/32 hours per week
