Case Manager I

Job Description: Facilitate the care of clients requiring follow-up services for abnormal breast and cervical results in accordance to program and MDHHS guidelines.

Responsibilities:

1. Responsible for follow-up care of all clients in the BCCCP, according to program and MDHHS requirements and guidelines
2. Responsible for the collection and entry of abnormal data into MBCIS in a timely manner according to program and MDHHS guidelines.
3. Responsible for the collection of all required data through providers, data retrieval systems and faxed reports.
4. Work closely with primary care physicians, surgeons, oncologists, labs and MDHHS to ensure timely follow-up.
5. Enter all necessary follow-up data into MBCIS in a timely manner according to program and MDHHS guidelines.

Qualifications:

1. Licensed to practice as a Licensed Practical Nurse (LPN) by the state of Michigan
2. Five years of medical hospital experience preferred.
3. Three years of case management experience preferred.
4. Oncology and GYN experience preferred.
5. Familiarity with chart review and data abstraction.
6. Computer and word processing skills and the ability to learn new computer applications, including simple databases and graphics.
7. Strong communication skills, both verbal and written.
8. Bilingual (Spanish and English) preferred
