Earn $23.50 as a BCM

Earn $24.60 as a Bilingual BCM

The Blended Case Manager (BCM) position is responsible for developing, coordinating, and implementing services to assigned consumers as identified in their service plan. The BCM will work and assist their assigned consumers by focusing on the following life domains:

• Behavioral and physical healthcare needs
• Family functioning/independent living
• Educational and vocational goals
• Socialization

The ideal candidate will possess the following:

• A bachelor's degree in psychology, social work, nursing, rehabilitation, counseling, or other related human services field
• Must possess a valid driver's license and have reliable transportation with consistent access to a vehicle during working hours to perform the essential duties of the position, including required travel between work locations.
• Experience in the human services field working with children is preferred
• Bilingual proficiency in English and Spanish is preferred
