Care Coordinator- Mount Sinai at Home- full time

As an integral part of a team, the Care Coordinator is responsible for supporting logistical patient care activities for patients cared for under Mount Sinai at Home. This role is responsible for performing various remote duties involving, but not limited to, providing support and assistance with orders necessary for home based care, coordinating with vendors and deliveries, ensuring patients receive necessary services based on care team orders. Role includes administrative tasks and requires clear communication and collaboration with clinical team, patients, vendors as well as other associated duties. Shares accountability for clinical quality and safety outcomes.
1. Works as a member of our interdisciplinary care team assessing and addressing the complex care needs of patients and families
2. Supports admission and ongoing home based census operations and follows up on orders to coordinate care
3. Provides administrative support for tracking ongoing care activities (vendor liaison, equipment pickup coordination, appointment scheduling, discharge order confirmation, etc)
4. Communicates plan of care and assessments to front line teams
5. Participates in huddles/IDT meetings daily/weekly with the rest of the care teams
6. Responsible for meeting with leadership on regular cadence (minimum monthly) to discuss Mount Sinai at Home quality metrics, provide feedback and solutions on program workflow expansion.
7. Manages and maintains operational database including relevant care coordination and care delivery related metrics
8. Serves as a liaison between physicians, clinical staff, and vendors
9. Sets priorities, problem solves and uses proper judgment in difficult situations.
10. Monitors Epic orders
11. Promptly answers calls from clinical teams.
12. Ensures that patient’s privacy is protected by adhering to MSHS information security policies and procedures and HIPAA guidelines.
13. Maintains accurate records of interventions and safety events.
14. Sets priorities, problem solves and uses proper judgment in difficult situations.
15. Submits utilization and assignment report every shift to supervisor and site leadership.
16. Maintains attendance/punctuality records in accordance to department procedure and/or policy.
17. Other assigned duties/responsibilities.
Experience Requirements
Minimum one year of healthcare experience including knowledge or experience with the following, three years preferred:
1. Experience with care coordination, patients, families, clinical staff
2. Ability to identify data elements to collect to support quality improvement/quality assurance.
3. Experience drafting organizational protocols and workflows.
4. Experience working with EMR, strong telephonic and coordination organization skills
5. Proven ability to identify strategies for measuring and communicating program successes and lessons learned.
Education, Licensing and Certification Requirements (if applicable)
1. Associate’s or Bachelor’s degree in health related field, required
2. Spanish speaking proficiency/fluency, preferred
3. Experience working as a healthcare paraprofessional, preferred.
4. BCLS, Preferred
