Under the direction of the clinic manager, or designee, the counselor shall assist in the personal, social and vocational rehabilitation of a patient being treated for substance abuse by providing supportive services either directly through individual and/or group counseling or by referral to outside agencies.
1. Screen and interview new patients, review records, and confer with other professionals to evaluate individuals opiate/substance use history, mental and physical condition and to determine their suitability for participation in treatment
2. Conduct chemical dependency program orientation sessions.
3. Counsel patients, individually or in group sessions, to assist in overcoming dependencies, adjusting to life, and working toward treatment goals.
4. Review and evaluate patients’ progress in relation to measurable goals described in treatment care plans.
5. Facilitate patient groups and documents the sessions in accordance with APG guidelines.
6. Gather psychosocial history and updates every five (5) years.
7. Develop individualized recovery plans: initial, interim, quarterly, semiannually and annually based on chemical dependency, fiscal, mental health, medical, vocational, employment and family concerns.
8. Maintain accurate patient records, in accordance with OASAS, NYC-DOH-MH, CSAT regulations and Joint Commission standards and guidelines.
9. Document all patient sessions following APG guidelines.
10. In collaboration with the interdisciplinary team, evaluates patient’s medication management, treatment concerns, and recovery plan updates, in multi-disciplinary team meetings (MDT).
11. Complete all other work assignments (financial updates, HRA/STARS reporting, family updates) within the required time frame.
12. Provide educational, HIV, family, vocational and fiscal treatment services, information and referrals for patients.
13. Maintain good community relations by participates in community tours to determine patient compliance with Mount Sinai Beth Israel policy and procedure.
14. Collect patient toxicology specimens as required by state law.
15. Refer patients to the appropriate agency to ensure adequate care of the patient needs.
16. Input patient data into HRA/STARS; and PAS 44, 45 and 26 into OASAS data banks
17. Receive weekly supervision.
18. Plan or implement follow-up or aftercare programs for clients to be discharged from treatment programs.
19. Acts as back-up counselor, when a counselor is absent from work.
20. Work Saturday Clinic as required.
21. Participates in the development of other staff members.
22. Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others
