Registered Nurse: Home Health Nursing

Applications are invited from suitably qualified candidates in Mercer County to fill the position of Registered Nurse: Home Health Nursing. This is a part-time position of 5-9 hours per week. No nights, no weekends, no holidays required. Presently, our Registered Nurses offer care to seniors enrolled in our Home Health Nursing Program everyone to two weeks. Duties: - Provide skilled nursing care to patients in their homes - Conduct comprehensive assessments of patients' health status - Develop and implement individualized care plans - Monitor and evaluate patients' progress and make necessary adjustments to the care plan - Educate patients and their families on managing their health conditions - Collaborate with other healthcare professionals to coordinate patient care - Document all patient interactions and interventions accurately and timely Requirements - Must be able to travel throughout Mercer County to see patients - Valid Registered Nurse (RN) license in the state of New Jersey - Two years home health nursing experience - Excellent communication and interpersonal skills - Compassionate, patient-centered approach to care Note: This job description is not intended to be all-inclusive.
