The Community Health Worker is part of an interdisciplinary care team that assists in care coordination, informing patients on community resources, and improving population health.

Responsibilities

→Performs cross continuum care activities, which may include coordinating care between primary care clinics, specialty care clinics, urgent care clinics, or other sites of care, as appropriate.
→Engages in outbound communication to patients and keep them engaged in their care, set-up appointments, and prompt them to act.
→Helps patients access community resources which may include scheduling appointments.
→Establishes contact with community partners, travel is required.
→Develop tools to educate peers on patient advocacy and community resources.
→Participates in community health fairs and events.
→Educates the care team on available community resources.
→Participates in quality improvement activities.

As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $24.83 - $35.71 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
