As a Life Coach (Agency with Choice) within our Family Services division, you will play a meaningful role in bringing that mission to life. Working one-on-one with individuals in community and home-based settings, you will provide personalized support designed to foster independence, build community connections, and help individuals work toward their personal goals. Responsibilities

→Provide individualized supports to clients for the purpose of fostering community inclusion, community building, and independence
→Assist clients with goal development, including long and short-term goals, and develop strategies for success
→Participate in ISP/LifePlan/progress meetings to support the individual in goal selection
→Create a schedule of appointments and meetings, as determined by the LifePlan/ISP and contract, that focus on the individual's outlined goals
→Assist individuals to interpret and respond appropriately to social cues and other situations encountered in the community
→Teach specific skills and behaviors, such as executive functioning skills and appropriate relational boundaries, in accordance with the goals, objectives, and strategies identified in the individual's plan
→Submit weekly documentation to the program coordinator on goal progress and maintain a log of contact and outreach in progress notes
→Provide mandated reporting consistent with Massachusetts General Laws, Chapter 119, Sections 51A-51G

Requirements

Education: Master's degree preferred
Experience: Five or more years of experience working with individuals with serious mental health issues and Autism, as well as a demonstrated working knowledge and background in developmental disabilities
Licensure: Preferred Licensed Independent Clinical Social Worker (LICSW) or Licensed Counselor
Skills and Knowledge: Excellent interpersonal and team-building skills for working with participants, coworkers, and client families; desire to help those who struggle with social pragmatics develop and maintain social-emotional competence and confidence; strong problem-solving skills; ability to adhere to deadlines for reports and progress notes; ability to maintain the rules of confidentiality in accordance with HIPAA
Driving Requirements: Valid driver's license and reliable personal vehicle required for transporting individuals to goal-related community activities, where applicable
Physical Demands: Must be able to stand for 80% of the workday and lift up to 40 pounds; ability to travel for work-related events
Training/Certifications: Must adhere to Universal Precautions at all times
Must successfully complete and pass all required background checks as a condition of employment
