Per Diem LPN

The Per Diem LPN is responsible to provide direct care to individuals living in a residential setting. He or she will follow the outlined plan of care, identify any health-related needs, monitor medical status and report changes to the on-call Registered Nurse, and work with residential staff to support the individuals in their community residence.

Schedule: Shifts Available including 9 am-3 pm, 3 pm-11pm, Choose one or more for our Danvers MA, Middleton MA and Topsfield MA

For our Burlington MA you must be available for the day shifts (730am-330pm) Monday - Friday.

• →Understand the clinical and social needs of the individuals’ receiving services.
• →Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care.
• →Assess individual throughout the shift to determine nursing needs. Report to and seek direction from the RN Case Manager or on-call RN as needed.
• →Present information regarding any individual in a professional manner in all settings including meetings, phone contact, daily written documentation and /or email correspondences.
• →Responsible to assist at physician's appointments, community trips, and other activities as assigned.
• →Prepare and teach health related in service to staff and provide ongoing supervision of ADL and other health related routines of each individual.

• Valid LPN license in Massachusetts.
• Experience with intellectual/developmentally disabled individuals a plus.
• Must have a valid Driver’s license and proof of a good driving record.
