Responsibilities

→Conduct comprehensive psychiatric evaluations and ongoing assessments in community-based settings
→Diagnose mental health conditions and develop individualized treatment plans
→Prescribe, monitor, and adjust psychiatric medications in accordance with best practices and regulatory standards
→Provide education to clients and families regarding diagnoses, medications, and treatment options
→Collaborate with ACT team members to coordinate holistic care, including medical, behavioral, and social supports
→Monitor client progress, response to treatment, and potential side effects
→Participate in daily team meetings, case conferences, and care coordination activities
→Provide crisis intervention and clinical support as needed
→Maintain accurate and timely documentation in compliance with agency and regulatory requirements

Requirements

Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP) in NYS
Master’s degree in Nursing required
Board certification as a Psychiatric Mental Health Nurse Practitioner
Minimum of 2 years of experience in psychiatric or behavioral health settings
Strong knowledge of psychiatric disorders, psychopharmacology, and integrated care practices
Excellent clinical judgment, communication, and interpersonal skills
Ability to work both independently and collaboratively within a multidisciplinary team

Experience working within an ACT team or community-based behavioral health program
Bilingual skills are a plus
Familiarity with electronic health records (EHR) systems
