Position: RN Case Manager
Location: Elmhurst, IL
Per Diem
Hours: Monday-Friday, 7:00-7:00p
Required Travel: To other Endeavor locations

Responsibilities

→Conduct comprehensive assessments of patients' medical, psychological, and social needs.
→Collaborate with physicians and interdisciplinary teams to develop personalized care plans.
→Identify potential barriers to care and proactively address them.
→Serve as the primary point of contact between patients, their families, healthcare providers, and community resources.
→Monitor and manage patient progress throughout their healthcare journey.
→Facilitate timely referrals to specialists, arrange for home healthcare services, and ensure seamless transitions during admissions and discharges.
→Educate patients and their families about their diagnoses, treatment options, and available support services.
→Promote patient self-management and empowerment to actively participate in their care decisions.
→Maintain accurate and up-to-date electronic health records, documenting all interactions, assessments, and interventions.
→Prepare reports and summaries to communicate patient progress and outcomes to the healthcare team.
→Collaborate with the interdisciplinary team to identify opportunities for improving care processes and patient outcomes.
→Participate in quality improvement initiatives and contribute to the development of best practices.
→Ensure compliance with all relevant healthcare regulations and standards, including HIPAA.

→Bachelors Degree Nursing Required Or
→Masters Degree Nursing Required
→2 Years of clinical nursing experience, preferably in an acute care or hospital setting.
→Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Current Registered Nurse (RN) license in the state of Illinois. Required And
→Certified Case Manager (CCM®) - Commission for Case Manager Certification (CCMC) Certification in Case Management (CCM) or related field Required
