As a Home Health PTA, you'll work closely with patients in their homes, helping them improve mobility, regain independence, and achieve their rehabilitation goals. You'll be supported by an experienced team of Physical Therapists, nurses, and healthcare professionals who share a commitment to delivering exceptional patient care.

• →Deliver physical therapy treatments and patient education under the direction of a licensed Physical Therapist and according to the established plan of care.
• →Help patients improve mobility, strength, balance, endurance, and functional independence.
• →Observe and report patient progress, responses to treatment, and any changes in condition to the supervising Physical Therapist.
• →Complete all clinical documentation accurately and within agency timelines.
• →Educate patients and caregivers on therapeutic exercises, fall prevention, safety techniques, and home exercise programs.
• →Collaborate with Physical Therapists, nurses, physicians, and other healthcare professionals to coordinate care.
• →Participate in care planning discussions and interdisciplinary team meetings as needed.
• →Follow all safety, infection prevention, and confidentiality standards.
• →Maintain compliance with agency policies and state and federal regulations.

• Active Illinois Physical Therapist Assistant (PTA) license.
• Graduate of an accredited Physical Therapist Assistant program recognized by the APTA.
• Minimum of six (6) months of PTA experience.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong communication, documentation, and interpersonal skills.
• Ability to travel throughout the Downtown Chicago service area.
• Ability to work independently while providing exceptional patient care.

• Home health experience.
• Experience working with geriatric and rehabilitation patients.
• Knowledge of Medicare home health documentation requirements.
• Experience with Axxess software.
• Bilingual candidates are encouraged to apply.
