We go beyond primary care with a full spectrum of services including cardiovascular health services, behavioral health, allergy testing and immunotherapy, in-house lab testing, imaging, chronic disease management, and other specialty health services. Responsibilities

  • Discuss and review patients' medical history, symptoms, allergies, and current medications
  • Perform comprehensive physical examinations and clinical assessments
  • Ask patients situation-specific questions to formulate accurate diagnoses
  • Diagnose and treat acute and chronic medical conditions
  • Develop and implement individualized treatment plans focused on improving patient outcomes
  • Prescribe suitable medications and provide proper dosage and administration instructions
  • Order, review, and interpret laboratory, imaging, and diagnostic test results
  • Provide preventive care services, wellness visits, health screenings, and patient education
  • Collaborate with care teams and specialists to coordinate comprehensive patient care
  • Manage patient panels and support value-based care initiatives, quality measures, and population health goals
  • Maintain accurate and timely documentation in eClinicalWorks (eCW)

Requirements

  • Doctor of Medicine (MD), Nurse Practitioner (NP), or Physician Assistant (PA)
  • Board-certified in Family Medicine, Internal Medicine, or a related field
  • Active Arizona license or ability to obtain prior to start date
  • Experience providing primary care services in an outpatient setting preferred
  • Sound medical knowledge
  • Experience utilizing electronic medical record systems, preferably eClinicalWorks (eCW)
  • Knowledge of value-based care models, quality measures, and population health management preferred
  • Excellent analytical and problem-solving skills
  • Exceptional communication skills
  • A patient and compassionate disposition
  • Detail-oriented
