Responsibilities

Referral Generation & Pipeline Development

  • →Lead outreach strategies to identify, engage, and refer ECM-, CS-, and BH-eligible members
  • →Set clear expectations for outreach staff related to referral targets and productivity
  • →Monitor referral volume, quality, and conversion rates
  • →Identify barriers to referral acceptance and implement corrective strategies

Community Partnership Development (Strategic Focus)

  • →Establish and sustain relationships with shelters, housing providers, street medicine teams, behavioral health providers, and community organizations
  • →Build strategic referral pipelines that support program growth and member access
  • →Position Pacific Health Group as a trusted partner within the local ecosystem
  • →Participate in coalitions, case conferences, and coordinated outreach meetings
  • →Formalize referral workflows and communication pathways with key partners

Staff Supervision & Performance Management

  • →Supervise and coach outreach staff with a focus on relationship-building and referral generation
  • →Train staff on effective community engagement and partnership development
  • →Conduct performance reviews tied to outreach productivity and outcomes
  • →Support team development and accountability

Outreach Operations

  • →Oversee daily outreach activities across hiring county
  • →Prioritize high-impact locations and partnerships that drive sustainable referrals
  • →Support staff in complex field-based engagement and enrollment efforts
  • →Ensure outreach coverage aligns with program growth and access goals

Program Integration (ECM, CS, Behavioral Health)

  • →Maintain strong knowledge of ECM eligibility pathways and Community Supports services
  • →Coordinate closely with care management and operational teams for seamless handoffs
  • →Align outreach strategies with health plan and county priorities

Cross-County & Health Plan Coordination

  • →Serve as the primary contact for outreach-related referral activity (if supporting additional regions or plans)
  • →Participate in performance reviews and partner meetings
  • →Support reporting, audits, and compliance requirements

Documentation, Data & Quality Improvement

  • →Ensure accurate documentation of outreach, referrals, and partnerships
  • →Analyze referral trends, partner performance, and outreach effectiveness
  • →Lead continuous improvement initiatives to strengthen referral pipelines

Requirements

Minimum Qualifications

  • Experience: 5+ years in community outreach, healthcare, social services, or related field
  • Leadership experience supervising field-based or outreach teams
  • Strong understanding of Medi-Cal, CalAIM, ECM, Community Supports, and Behavioral Health programs
  • Proven experience building and maintaining community partnerships
  • Strong communication, leadership, and relationship management skills
  • Ability to manage multiple priorities and partnerships effectively
  • Strong technical proficiency, including CRM systems, reporting tools, and documentation platforms
  • Ability to analyze data and translate insights into actionable strategies

Work Requirements

  • Ability to travel extensively within hiring county
  • Ability to work occasional evenings or weekends for community events
  • Ability to work in a hybrid, field-based leadership role
  • Reliable personal vehicle for daily work use
  • Successful completion of background check (including MVR)|
  • Must be able to travel up to 60-70% within the county to conduct in person visits
  • Must successfully complete a Testlify skills assessment
  • Must have a reliable working laptop for the first 21 days of employment (personal equipment stipend) until company
    issues laptop is received
  • Must have effective Time Management skills
  • Must have internet speed of - 300+ mbps download and 25+mbps upload
  • Must be proficient in technology, including documentation systems, case management platforms, and communication tools
  • Must be willing to work occasional evenings and weekends as needed.
