Licensed Practical Nurse (Hospice)

• Service Area: Westwood, Quincy, Brockton, Walpole, Marshfield, Weymouth, Holbrook, Randolph areas.
• Job Type: Per Diem

• →Provide skilled nursing care under the supervision of a Registered Nurse and in accordance with the plan of care.
• →Monitor and document patient condition, vital signs, symptoms, and responses to treatment.
• →Administer medications and treatments as prescribed.
• →Assist with implementation and ongoing evaluation of patient care plans.
• →Educate patients and caregivers on medications, treatments, and disease management.
• →Communicate changes in patient condition to the supervising RN and physician as appropriate.
• →Maintain accurate and timely clinical documentation.
• →Promote a safe and comfortable environment for patients.

• →Coordinate necessary supplies, equipment, and services for patient care.
• →Support quality improvement initiatives and compliance standards.
• →Accept and fulfill assigned patient visits and schedules reliably.
  
  

• Current and active LPN license in Massachusetts.
• Graduate of an accredited practical nursing program.
• Previous nursing experience preferred, especially in home health, hospice, or geriatrics preferred.
• Proficient with electronic medical records (EMR).
• Excellent communication and interpersonal skills.
• Current BLS CPR certification.
• Valid driver’s license and reliable insured vehicle required.
