Requirements

Current APRN state professional license (Adult/Geriatrics specialty preferred), DEA certificate, CPR certification.
Master’s degree in Advanced Practice Nursing from an accredited school.
At least 2 years as a Nurse Practitioner, preferably in home health care.
Valid driver’s license with reliable transportation.
Excellent communication, problem-solving, and organizational skills. Homecare Homebase knowledge is a plus.

Responsibilities

→Conduct admission assessments and develop personalized treatment plans.
→Provide routine and urgent care visits, ensuring flexibility to meet patient needs.
→Manage medication orders and educate patients on proper usage.
→Handle wound care, testing, and equipment/supply needs.
→Document patient progress and collaborate with the care team for optimal results.
→Assist in discharge planning and coordinate with community resources.
→Participate in team meetings and briefings to ensure cohesive patient care.
→Maintain patient confidentiality and report any incidents or safety concerns.
