• Service Area: Boxboro, Harvard, Littleton, Maynard, Stow, and surrounding areas.
• Job Type: Per Diem

• →Initiate appropriate preventive and rehabilitative nursing procedures.
• →Administer medications and treatments as prescribed by physicians.
• →Conduct initial assessments of patients and families to determine home care needs.
• →Perform thorough physical assessments and gather comprehensive medical histories.
• →Develop and revise individualized plans of care based on patient status and evolving needs.
• →Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• →Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• →Provide counseling and support to patients and caregivers before, during, and after treatment.
• →Supervise home health aides as required.
• →Identify discharge planning needs and proactively implement them as part of the care plan development.

• Must hold an active Nursing license in the State where the position is based.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
