Community Outreach Specialist (Full-Time)

The Philadelphia Department of Public Health (PDPH), Division of Disease Control (DDC), works to prevent, control, and report on diseases and conditions that are contagious and/or affect the public’s health. DDC helps prepare for public health emergencies and educate the community on how to stay safe and healthy.
The Philadelphia Department of Public Health (PDPH), through its Immunization Program in the Division of Disease Control, oversees the purchase and distribution of federally funded vaccines to local healthcare providers. As one of 64 U.S. Centers for Disease Control and Prevention (CDC) funded program areas, the Philadelphia Immunization Program is charged with the mission to prevent vaccine-preventable diseases and increase the immunization coverage levels among infants, children, adolescents, and adults in Philadelphia.
The position will conduct outreach activities with the public and community-based organizations. This position is also responsible for building and maintaining relationships with stakeholders and tracking progress. This position will involve routine and frequent fieldwork, with in-person support at community events, health fairs, and outreach response activities. This is not remote work. This position will work from the Health Department offices or other locations as assigned. This position is Monday through Friday, 8:30 a.m. to 5:00 p.m., with regular work after hours and on weekends to support Program activities. This is a grant-funded position; continuation of the position is contingent upon sustained funding.
Responsibilities

- →Become proficient in immunization and vaccine information and be able to communicate key messaging to residents.
- →Develop/maintain partnerships with community-based partners, faith-based organizations, service provider agencies, and other stakeholders to ensure effective two-way communication with a variety of audiences based on stakeholder information needs, concerns, and questions.
- →Identify and conduct outreach to partners who serve high-risk communities and manage relationships with block captains, faith leaders, and other trusted messengers.
- →Collect input from partners and liaise with stakeholders to ensure the information needs of the whole community are met.
- →Track engagement activities using the program database.
- →Coordinate with the Communications team to provide feedback on messaging and to disseminate educational materials and messaging throughout the community.
- →Identify effective channels of communication for reaching target audiences, including identification of creative, non-digital methods to promote important preparedness and emergency response information to at-risk or socially isolated populations.
- →Utilize the PhilaVax Immunization Registry for data entry and tracking purposes.
- →Enter data, track cases, disposition cases in the registry.
- →Participate in Immunization Program activities including trainings, exercises, and real public health response activities.
- →Other duties as assigned.
- Fluency in languages other than English is desirable.
- Working knowledge of Microsoft Office applications.
- Strong written and verbal communication skills, good common sense and judgment.
- Detail-oriented with excellent organizational skills.
- Ability to communicate effectively and work with various teams.
- Flexibility, professionalism, and the ability to manage multiple projects simultaneously are highly valued attributes.
- A valid driver’s license is required.
3-5 years of previous experience in community-based outreach and engagement, health education, and/or partnership building preferred, with experience engaging with community members of diverse backgrounds.
