→Ability to work in a multi-disciplinary team and handle complex discharge planning
→Strong communication skills and ability to work under pressure
→Ability to contribute to a 7-day service, including public holidays
→Enhanced DBS check required
→Previous experience working within the NHS is highly desirable.
