Band 7 Pharmacist - Hospital & Prescribing - Birmingham

• →Provide clinical pharmacy services across designated wards and departments.
• →Support safe and effective prescribing practices throughout the hospital.
• →Undertake medicines reconciliation and clinical medication reviews.
• →Provide specialist pharmaceutical advice to medical, nursing, and allied healthcare professionals.
• →Promote medicines optimisation and support the safe use of medicines.
• →Contribute to discharge planning and continuity of care initiatives.
• →Participate in clinical audits, governance activities, and service improvement projects.
• →Maintain accurate and compliant clinical documentation.

• Be registered with the General Pharmaceutical Council (GPhC).
• Have previous NHS hospital pharmacy experience.
• Demonstrate experience working at Band 7 level or equivalent.
• Possess strong clinical knowledge and prescribing support experience.
• Have experience undertaking clinical medication reviews and medicines reconciliation.
• Be confident working independently and within multidisciplinary teams.
• Demonstrate excellent communication, organisational, and problem-solving skills.
• Independent Prescriber qualification is desirable but not essential.
