Are you a dedicated nurse ready to make a meaningful impact in rural and regional communities? Join Country Health Connect as a Community Health Nurse (Registered Nurse) and help deliver quality, person-centred care across the Limestone Coast region.
In this rewarding casual pool role, you’ll be able to work across all Country Health Connect sites in the Limestone Coast, supporting clients in their homes and communities. You’ll assess patient needs, plan and coordinate services, and provide expert nursing care that improves lives. Working in a flexible, multidisciplinary model, you’ll partner with consumers, communities and fellow health professionals to deliver compassionate care where it’s needed most.
Opportunities exist to extend your skills in the Immunisation Program, Wound Care and Continence services.
Responsibilities
~1 min read- →Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate
- →Ideally, you have experience in community health
- →An ability to work within a multidisciplinary team.
- →The ability to engage with patients in a culturally appropriate way is a must, and you’ll have a flexible, innovative approach to problem-solving.
- →A desire to work towards becoming an Authorised Immunisation Nurse
Country Health Connect focuses on the delivery of exceptional care to those living within our communities who require some support to remain living independently or reside in one of our residential aged care facilities.
Our professional and caring staff deliver health care and wellbeing support across regional South Australia in hospitals, local health services, community centres, residential aged care facilities and clients’ own homes.

- National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups required for this position (E.g. aged care employment or work involving vulnerable clients)
