Responsibilities

  1. →Maintain, organize, update, and retrieve patient medical records in electronic and paper-based systems, as applicable.
  2. →Review medical records for accuracy, completeness, and proper documentation.
  3. →Scan, index, upload, and file medical documents into the appropriate patient records.
  4. →Process requests for medical records from patients, healthcare providers, insurance companies, attorneys, and other authorized parties in accordance with established procedures.
  5. →Verify patient identities, authorization forms, and release-of-information requests before disclosing protected health information.
  6. →Ensure the confidentiality, security, and integrity of patient information in accordance with HIPAA and applicable federal and state privacy laws.
  7. →Track and follow up on missing, incomplete, or outstanding medical documentation.
  8. →Assist with medical record audits, quality assurance reviews, and compliance activities.
  9. →Maintain accurate records of document requests, releases, and other medical records transactions.
  10. →Communicate professionally with clinical staff, administrative personnel, patients, and external organizations regarding medical record inquiries.
  11. →Identify and report documentation discrepancies, filing errors, and potential privacy or security concerns
  12. →Perform other related duties as assigned.
