A remote, full-time W-2 clinical pharmacist role leading GDMT optimization and medication titration in partnership with nurses and cardiology providers.

Role Overview
Our client is hiring a full-time, W-2 Clinical Pharmacist with deep cardiology experience to support its value-based care programs. The heart of this role is guideline-directed medical therapy (GDMT): getting high-risk cardiac patients onto the right therapies, at the right doses, and keeping them there. You will work alongside RN Care Managers, Nurse Navigators and cardiology providers to close therapy gaps, support titration and improve adherence.
This is not a dispensing role. You will be a clinical partner embedded in the care team, and your recommendations will directly shape patient outcomes and quality performance.
Key Responsibilities
GDMT Optimization & Medication Management
Review medication profiles for cardiology populations, including heart failure, coronary artery disease, atrial fibrillation and hypertension, and overlapping conditions such as CKD.
Lead GDMT assessment against ACC/AHA guidelines and payer quality programs. For HFrEF, that means evaluating and optimizing the four foundational therapy classes: ARNI/ACEi/ARB, evidence-based beta-blockers, MRAs and SGLT2 inhibitors.
Identify therapy gaps, missed titration opportunities, contraindications, drug interactions and adherence barriers.
Make evidence-based recommendations for initiation, dose optimization and titration, in collaboration with providers and nursing staff.
Apply sound clinical judgment within established clinical protocols.
Patient Engagement
Conduct telephonic medication reviews and patient education.
Explain the purpose, benefits, side effects and expectations of cardiac medications in clear, patient-friendly language.
Reinforce adherence and lifestyle considerations, and help patients recognize red-flag symptoms.
Care Team Collaboration
Partner with RN Care Managers and Nurse Navigators to align medication plans with care goals.
Communicate recommendations clearly and confidently to cardiologists and advanced practice providers.
Participate in interdisciplinary case reviews and care planning.
Document recommendations and patient interactions promptly in the care management platform / EMR.
Quality & Value-Based Care
Contribute to performance on medication-related quality measures.
Help reduce medication-related adverse events and avoidable utilization.

A clinically confident cardiology pharmacist who makes GDMT decisions with ease, works well by phone, explains complex therapy in plain language, and does well in a nurse-led, interdisciplinary care model.
Strong cardiology background with hands-on GDMT decision-making and titration experience.
Confident, professional communicator with cardiologists and APPs.
Patient-centered style: clear, empathetic and clinically accurate.
Comfortable in a fully remote, telephonic, non-dispensing clinical role.
Experience & Skills
Required:
PharmD
Active, unrestricted Texas pharmacist license in good standing. [Choose one: "required at time of hire" / "required before start date. Pharmacists licensed in another state who are eligible for Texas licensure by transfer are encouraged to apply, provided the Texas license is secured before the start date."]
3–5+ years of clinical pharmacy experience with a strong cardiology focus
Demonstrated, hands-on experience with GDMT, especially HFrEF therapy optimization and cardiac medication titration
Working knowledge of ACC/AHA guidelines
Comfort working within a nurse-led care management model
Strong telephonic patient communication skills
Professional, confident collaboration with physicians and advanced practice providers
Time zone compatibility: able to work Central Time business hours
Strong written and verbal communication: exceptional written English, assessed at screening
AI fluency: comfortable using AI tools to work faster and more accurately, with specific tools and use cases confirmed at screening
Preferred:
Experience in heart failure or cardiology clinics
Experience in clinically integrated networks or value-based care models
Familiarity with payer quality programs
Experience documenting in EMRs or care management platforms
Prior remote or telehealth patient engagement experience
