• Support community nurses and care teams with resident care.
• Complete assessments and follow up on physician orders.
• Assist with admissions, documentation, and care coordination.
• Provide guidance and support to caregivers.
• Help maintain high standards of clinical care.

You're the extra support that helps our teams shine and our residents thrive.

• Current Minnesota RN license.
• Positive attitude and passion for serving others.
• Adaptable, dependable, and comfortable working independently.
• Assisted living or memory care experience is a plus.
• Willing and able to travel between communities within 25 miles of your residence.
