It is SOAHAC’s objective to build health care capacity within Indigenous communities   The Opportunity    SOAHAC is seeking a Harm Reduction Care Coordinator to join our interdisciplinary team at the Chippewa of the Thames First Nation Site.    Reporting to the Integrated Care Manager, the Harm Reduction Care Coordinator provides culturally safe, client-centered support to Indigenous individuals and families impacted by substance use. Working within an interdisciplinary team, the successful candidate will support clients through care coordination, advocacy, system navigation, harm reduction education, crisis support, and connections to health, wellness, cultural, and community services.    This role is ideal for someone passionate about harm reduction, Indigenous wellness, relationship-building, and helping clients navigate complex health and social systems.   Responsibilities: Provide harm reduction support, care coordination, advocacy, and system navigation to Indigenous individuals and families.  Conduct intake, assessment, referrals, and individualized wellness planning.  Promote harm reduction, overdose prevention, health education, and wellness initiatives.  Collaborate with Primary Care, Mental Health, Traditional Healing, and community partners to support coordinated client care.  Support access to substance-use treatment, health services, cultural supports, and community resources.  Build and maintain positive relationships with Indigenous communities, organizations, and service providers.  Participate in outreach, health promotion, and community engagement activities.  Maintain accurate client documentation, statistics, and reports.  Contribute to program planning, evaluation, and continuous quality improvement initiatives.  Participate in staff meetings, training opportunities, cultural teachings, and organizational activities as required.     Qualifications  Post-secondary degree or diploma in health, social services, addictions, mental health, Indigenous studies, community services or a related field, with a minimum of one year of relevant experience, or an equivalent combination of education and experience.  Experience working with individuals who use substances and/or populations experiencing barriers related to homelessness, poverty, mental health, street involvement or other forms of marginalization.  Knowledge of Indigenous harm reduction philosophies and principles and substance-use care.  Understanding of trauma-informed, strengths-based and culturally safe approaches to care.  Knowledge and understanding of Indigenous approaches to health and wellness and the impacts of colonization and systemic barriers on Indigenous health.  Demonstrated crisis intervention, de-escalation, advocacy and relationship-building skills.  Strong interpersonal, verbal, written and organizational skills.  Ability to work independently and collaboratively within an interdisciplinary team.  Commitment to traditional and western approaches to Indigenous health and wellness.    Requirements: Willingness to learn Indigenous Cultural Safety and other ongoing cultural learnings/ teachings Willingness to assist other SOAHAC sites as needed Valid driver’s license or access to a reliable transportation Ability to obtain/ maintain a current and clear police check is a condition of employment Up-to-date immunizations and records This job may require additional responsibilities and duties as assigned by Southwest Ontario Aboriginal Health Access Centre.   SOAHAC values diversity and is an equal opportunity employer; however, hiring preference will be given to qualified Indigenous applicants (please self-identify).  SOAHAC is committed to providing employment accommodation in accordance with the Ontario Human Rights Code and the Accessibility for Ontarians with Disabilities Act. If you require accommodation to apply or if selected to participate in an assessment process, please advise Human Resources.   FT Harm Reduction Care Coordinator | Dayforce Jobs   Please apply by submitting your resume and cover letter.
