Home Health Occupational Therapist

- Conduct assessments to identify patients' needs and develop customized treatment plans

- Implement therapeutic activities to improve motor function, coordination, and mobility

- Educate patients and their families on adaptive techniques and equipment

- Collaborate with healthcare professionals to ensure comprehensive care

- Maintain detailed patient records and progress reports on day of the visit

- Bachelor's or Master's degree in Occupational Therapy

- Active state licensure as an Occupational Therapist

- Experience with home health / geriatric community preferred

- Excellent communication skills and empathy towards patients

- Detail-oriented with strong organizational abilities
