Licensed Practical Nurse (LPN) Full-Time

St. Joseph’s Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our nursing team in a community-based Intermediate Care Facility home. This home supports 6 residents, providing a safe and nurturing environment where dignity and independence are prioritized.

• →Provide direct nursing care and health monitoring for residents.
• →Administer medications accurately and monitor for side effects.
• →Develop, implement, and evaluate individualized care plans.
• →Maintain medical records and ensure compliance with ICF/ID regulations.
• →Assist residents with activities of daily living (hygiene, dressing, feeding).
• →Accompany residents to medical and dental appointments.
• →Respond to health emergencies and implement appropriate interventions.
• →Collaborate with the interdisciplinary team and participate in care planning.
• →Maintain adequate medical supplies and equipment.
• →Support residents in community activities and outings.

Competitive Pay - Starting at $27.50/hour + $2.00/hour shift differential, $5,000 sign-on bonus

• Current LPN license in Pennsylvania
• CPR certification
• Valid PA driver’s license
• Experience with individuals with intellectual and developmental disabilities preferred
• Ability to lift/move 25–50 lbs and assist with transfers
• Strong communication and teamwork skills
