At St. Joseph's Home Care we’re dedicated to delivering compassionate, high-quality care to clients who have been discharged from hospital but who are unable to return to their pre-hospital living arrangements. These clients are in process of having long term care needs assessed, are generally medically stable, can be of any adult age group / elderly age group, and may require mobility assistance. Client stays are usually short term in length, and they may require a care plan.

As a Personal Support Worker, you’ll play a vital role in helping clients with personal care assistance, medication reminders, physical and cognitive support and home keeping assistance. PSW-HALP assists in enabling individuals to continue living independently in a safe and healthy manner in their own home/apartment units, within the various Halton Assisted Living program locations and the surrounding community.

This role is ideal for a compassionate, innovative, and collaborative professionals who thrives in a dynamic home and community care setting.

Responsibilities

CORE DUTIES AND RESPONSIBILITIES

→Assisting clients with daily personal care activities, in keeping with their individual care plan
- →Bathing
- →Dressing/undressing
- →Personal hair care
- →Shaving/grooming
- →Mouth care
- →Preventative skin care
- →Other personal hygiene tasks
→Assisting clients with daily housekeeping activities, in keeping with their individual care plan
- →Light apartment housekeeping (sweeping, tidying, dishes etc.)
- →Laundry services
- →Light meal preparation / safe food handling
→Assisting clients in daily activities which maintain adequate levels of physical health and safety, in keeping with their individual care plan
- →Performing safety wellness checks
- →Medication reminders
- →Living area visual scans
- →Engaging clients in positive/social conversations
- →Encouraging clients to direct and/or participate in their own care
- →Assisting clients with general mobility needs (walking, movement, positioning etc.) in and around their home and the program location
- →Encouraging participation in HALS social and/or recreational activities
- →Assisting with basic range of motion exercises where required by the care plan
- →Demonstrating sensitivity and respect to clients who may be experiencing physical health challenges
- →Providing emotional and social support to both clients and their family members
- →Demonstrating a positive attitude and approach towards clients as unique individuals
→Performs daily work in keeping with the established Assisted Living procedures, processes and expectations
- →Answers call bells in a safe and timely manner
- →Observes and records client information/comments/issues/behaviours/changes on the 24-Hour Shift Report
- →Follows daily client care schedules as determined by the Manager or Supervisor
- →Maintains a clean and organized work environment
- →Reports critical client care information/issues to Manager or Supervisor right away
- →Understands and follows individual client care plans, seeks clarification from the Manager or Supervisor where care plan in unclear
- →Participating in team/group meetings
- →Other duties as requested or required

WHAT YOU BRING

Education & Credentials:

→PSW Certificate from an accredited training institution (Required)
→Member of the HSCPOA (preferred) May be required under some contracts
→Current CPR First Aid certification
→Training in Gentle Persuasive Approaches / non-violent crisis prevention (preferred)
→Access to reliable transportation, drivers liscense and vehicle insurance, required
→Vulnerable Police Sector Check provided at time of hire, annual declaration (required)
→2 step TB test at time of hire (required)

Experience:

→1–3 years of clinical experience (required)
→Training and previous work experience with medication reminders (preferred)
→Previous experience working in an Assisted Living environment (preferred)
→Home and community care experience (preferred)
