Community Health Worker -- Full time -- Days

St. Vincent's Hospital Westchester is seeking Community Health Workers to join our Care Coordination program. This program provides support and coordination services to adults in the community who struggle with medical and behavioral health conditions. The Community Health Worker will assist clients with concrete needs, including access to transportation, housing support, and food and nutrition support. Reporting to the Team Leader, the Community Health Worker collaborates with clients and their community supports to develop personalized service plans, and ensures that clients receive concrete services in a

timely manner.

Job responsibilities include:

• Complete screening and assessments with eligible clients.
• Develop service plans to address identified needs.
• Document services according to program and billing standards.

Qualified candidates will have excellent communication and organization skills.

Requirements: Bachelor's degree and 1-2 years' experience in a social service or health care setting.

Pay rate: 1199 union rates
