Synapticure is the largest, most comprehensive specialty care and life sciences company in the country for those living with neurodegenerative diseases. Founded by patients and caregivers living with these diseases, we are redefining the care and the research paradigm for conditions like Alzheimer's, Parkinson's, and ALS in all 50 states.

Our business is powered by two connected engines.

A scalable, 50-state virtualized clinical network providing timely access to expert neurologists and cutting edge treatments with wraparound care coordination and behavioral health support. Our team creates personalized care plans focused on education and empowerment, connecting patients with a multidisciplinary team of subspecialty neurologists, geriatricians, psychologists, and care coordinators who collaborate to deliver comprehensive care.

Leveraging our deeply engaged, well characterized national patient cohort, CURES partners with life science, pharma, biotech, and medical device companies to accelerate the development of neurodegenerative treatments, spanning discovery stage lab services, clinical trial services, and real world evidence data and analytics.

Partnering with providers, health plans, and life sciences companies, including CMS' GUIDE dementia care model, Synapticure is dedicated to transforming the lives of millions of individuals and their families living with neurodegenerative diseases.

Synapticure is seeking an experienced and compassionate Cognitive Advanced Practice Provider, Nurse Practitioner (NP) or Physician Assistant (PA), to join our growing Cognitive Care team. In this fully remote role, you will provide comprehensive telehealth care to patients living with Alzheimer’s disease and other dementias by partnering closely with cognitive neurologists, geriatricians, behavioral health clinicians, and care coordinators to deliver coordinated, high-quality virtual care.

You will diagnose, treat, and manage dementia and cognitive disorders while supporting patients and caregivers through education, medication management, and care planning. The ideal candidate has a deep understanding of dementia and cognitive medicine, experience supporting patients with complex neurodegenerative diseases, and the ability to thrive in a fast-paced, technology-enabled, team-based environment.

  • Conduct comprehensive telehealth visits for patients with cognitive and memory disorders, including Alzheimer’s disease, vascular dementia, Lewy body dementia, and frontotemporal dementia
  • Diagnose, treat, and manage cognitive impairment and behavioral symptoms through evidence-based approaches and ongoing monitoring
  • Collaborate with neurologists, geriatricians, behavioral health clinicians, and interdisciplinary team members to create and implement individualized dementia care plans
  • Perform annual care plan reviews, monitor disease progression, and modify treatment plans as needed based on patient and caregiver feedback
  • Provide education, coaching, and behavioral management strategies to caregivers and families to improve care quality and reduce distress
  • Order and interpret appropriate laboratory and diagnostic tests, adjusting treatment plans in alignment with best practices and patient goals
  • Facilitate coordination with specialists and community resources to ensure comprehensive, continuous care
  • Participate in on-call rotation during daytime hours to triage patient concerns and support urgent issues
  • Maintain accurate, timely documentation in the electronic health record (EHR) and ensure compliance with all federal, state, and program guidelines
  • Support Synapticure’s CMS GUIDE dementia care initiatives through adherence to program workflows, data collection, and quality improvement activities
  • Contribute to ongoing education and mentorship for care navigators, caregivers, and clinical colleagues on dementia care best practices
  • Collaborate with leadership to optimize clinical operations, patient satisfaction, and performance outcomes
  • Active, unrestricted NP or PA license (APRN with ANCC/AANP certification, or PA with NCCPA certification) and completion of an accredited Nurse Practitioner or Physician Assistant program (MSN, DNP, or PA equivalent)
  • Minimum of 2 years of clinical experience as an NP or PA within geriatrics or neurology
  • Strong preference for direct experience in dementia and cognitive care
  • Proven ability to assess, diagnose, and manage patients with cognitive impairment, dementia, and related neurodegenerative conditions
  • Working knowledge of geriatric medicine, behavioral and psychological symptoms of dementia (BPSD), and multidisciplinary care coordination
  • Proficiency with telehealth technologies, virtual visit platforms, and EHR documentation
  • Excellent interpersonal and communication skills, with the ability to provide empathetic, patient- and caregiver-centered care
  • Ability to balance clinical responsibilities, patient education, and collaboration with remote teams
  • Strong organizational and time-management skills in a fast-paced clinical environment
  • Commitment to continuous learning, evidence-based practice, and quality improvement
  • Prior experience in a dementia or cognitive neurology program, memory care clinic, or GUIDE-model program.
  • Clinical research experience related to Alzheimer’s disease or dementia treatment.
  • Familiarity with amyloid targeting therapies and other emerging Alzheimer’s treatments.
  • Bilingual fluency in English and Spanish.
  • Experience providing caregiver training and community resource navigation.
