Certified Peer Support - Recovery Facilitator - Willowbrooke (Evenings)

The Recovery Facilitator's role is to provide skills training and support as part of the Willowbrooke at Tanner health system and to promote self-determination, personal responsibility and empowerment inherent in self-directed recovery. Under immediate to general supervision, the Recovery Facilitator provides recovery support services and serves as a consumer advocate and provides consumer information and peer support for consumers in a variety of settings. The employee performs a wide range of tasks to support consumers in living their own lives and directing their own recovery and wellness process. The facilitator will model competency in recovery and wellness.

Area of Responsibilities
*Communication Team work Conduct- Position requires frequent contact with others both internal and external to the facility, the employee must: communicate and work effectively with others to promote teamwork. Employee must exhibit the tanner standards of behavior. maintain confidentiality in compliance with the standards and communicate with patients, visitors, and customers in a manner which promotes patient satisfaction. Establishes and maintains effective working relationships with supervisor, coworkers and staff of other departments. Establishes and maintains positive relationships with consumers, physicians, allied health professionals and all referral sources.
*Documentation- Documents calls and related information on designated forms. Notates all necessary information regarding discharge planning in order to facility follow through with discharge plans. Document any contacts with patient, treatment team, and/or any relevant collateral contacts involved with the patient.
*Participation in the General Departmental Operation- Stays abreast of organization's programs and services. Supports organizations programs and services. Supports organizations customer service philosophy.
*Provision of Care- Attending treatment team and discharge planning meetings at all levels of care within the Willowbrooke continuum. Providing case management for patients whose discharge plans include transitioning to a different level of care within the Willowbrooke continuum. Monitoring and follows-up to determine if the recommended services have adequately met the consumer's needs. Working to ensure all Willowbrooke internal referral a successfully admitted in to the recommended level of care. Communicates barriers following discharge recommendations to the appropriate clinical staff. The position requires discretion, tact and diplomacy to facilitate the coordination and continuity of care and to maintain communication for the coordination of benefits and resources. Establishes and maintains positive relationships with patients, physicians, allied health professionals, and all referral sources. Works cooperatively with Program Manager in program development and facilitates skill and support groups for identified patients at various levels of care. 
*Standards Policy and Procedure- Stays abreast of organization's programs and services. Supports organization's customer service philosophy. Responsible for completion of required facility and departmental education under the direction of the department manager. Complies with other applicable department and/or organization standards and policies. Complies with facility safety, infection control and security program. Supports facility internal and external customer service standards. Responsible for knowledge of policies and procedures and standards related to the provision of care. including The Joint Commission, Infection Prevention, billing and treatment regulations set forth in the Community Mental Health, Developmental Disabilities and Addictive Diseases Provides for the Department of Behavioral Health provider manual. Responsible for completion of required facility and departmental education under the direction of the department manager. Employee will obtain initial BLS and SCIPP certification within 90 days of employment. Employee will maintain BLS and SCIPP certification as required.

Compliance Statement
Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.
