Responsibilities

→Welcoming people home from hospital and helping them settle in safely
→Supporting personal care, meals, medication prompts and daily routines
→Helping people rebuild their confidence and practise everyday skills, in line with their reablement plan
→Spotting any changes in someone's health or wellbeing and reporting them promptly
→Keeping clear, accurate records and working closely with the wider team and health professionals

You hold a full UK driving licence and have access to your own car with business insurance.
You’re available to work shifts, including early mornings, evenings and weekends.
You’re willing to undergo an enhanced DBS check, which we’ll arrange.
